Travel history survey in response to 2019-nCoV outbreak

Dear students
 
In response to the severe situation of the outbreak of 2019 Novel Coronavirus (2019-nCoV) in China. You are strongly advised NOT to fly to Taiwan via China, HK or Macau. Please fill in the questionnaire as soon as possible. Please note that this is mandatory and every student shall submit this questionnaire before Feb 11th 12:00 p.m. lunch time.

In its efforts to prevent the novel coronavirus (2019-nCoV) infection, the Ministry of Education, R.O.C has notified all schools and universities in Taiwan that foreign students, who have visited Mainland China, Hong Kong and Macau shall stay under home quarantine for a period of 14 days after returning to Taiwan. A fine of NTD 10,000 to NTD 150,000 will be imposed on students who violate home quarantine regulations by going out or taking public transportation, according to the ‘Communicable Disease Control Act’ Article No. 58 and Article No. 69.
Please fill in the questionnaire above and stay alert when you travel to Taiwan. Taiwan Tech will try its best to help.
 
For university dormitory boarders, who have travelled via China, Hong Kong, or Macau (including transfer), are required to enter Taiwan before Feb 15th.  Taiwan Tech will provide assistance in finding a place for home quarantine. For those who arrive after Feb 15th, students will have to find qualified quarantine places and bear all the expenses on their own.
 
For foreign students with a travel history of China, Hong Kong, or Macau (including transfer), who do not reside in an on- campus dormitory will have to find qualified quarantine places and bear all the expenses on their own.
 
According to regulations of the Central Epidemic Command Center (CECC), individuals (exclude students from China, Hong Kong and Macao who are not allowed to enter Taiwan) who wish to enter Taiwan and have a recent travel history to China, Hong Kong and Macau, will be required to stay under home quarantine for 14 days starting from February 7th. 
 
Students, who visited China, Hong Kong or Macau (including transfer), should prepare a sufficient supply of surgical face masks for the 14-day home quarantine before entering Taiwan.
 
Students living off-campus should follow the regulations by Taiwan Centers for Disease Control and quarantine themselves in their homes.
